  Please answer the following questions on separate paper. (Typed answers are preferred.)
  
  
  1. Why do you wish to attend The Outdoor Academy, and what do you hope to gain from the experience?
  2. What book has made an impression on you and how?
  3. What is one thing you want to accomplish in your life and why?
  4. List any activities in which you are involved outside the classroom, including volunteer work or community service. Which ones are the most important to you and why?
  5. What was a challenge that you have had in your past and how did you work to overcome it?
      
• Creative Presentation 
  Tell us more about yourself through a creative presentation.  Please submit original works of poetry, visual art or video that tells us about your hobbies, talents, skills and interests.
  
  
  1. Written and visual art pieces should fit into a 9" x 12" envelope.
  2. Videos should be sent on CD and should not exceed 5 minutes in length.
      
• A copy of a graded paper you have written recently for your English or history class
• A copy of the applicant’s school transcript (grades from the past 2 years)
• Two sealed teacher recommendations (one primary, one secondary)
• Completed OA Financial Aid Form and SSS Financial Aid Forms, if applicable.
