2023 Added Adventure Participant Questionnaire

Parent or Guardian, please have your participant complete this form. Your camper's Added Adventure Application is not complete until we have received this form as well as the Parent/Guardian portion in CampDoc under Applicant Information. An Added Adventure is an excellent opportunity to explore the natural world outside of Eagle’s Nest Camp. Using your and your parent/guardian answers to these questionnaires, we will be choosing a small group of campers to go on this adventure. Take a little time to imagine what you think this trip might be like and why you think it would be a good adventure for you. As you answer these questions, imagine the feeling of warm sun on your back or the starry skies over your camp-site.
