2025 Hante Participant Questionnaire

Parent or Guardian, please have your participant complete this form. Your participant's Hante Application is not complete until we have received this form as well as the Parent/Guardian portion in CampDoc under Applicant Information. Participating in a Hante course is an excellent way to “learn by doing.” In order for you to be considered for selection, please answer the following questions honestly. Your application is not complete until we receive a completed application on CampDoc, the upload of the Participant Questionnaire on CampDoc, and a completed Parent/Guardian Questionnaire on CampDoc. Completed applications for Hante Adventures are due by November 15th.
